Specialty, state, payer, and age rules can add forms, so the clinic manager should maintain a service-specific packet for each appointment type.

Can a clinic treat a patient who will not sign the privacy notice acknowledgement?

A patient's refusal to sign the acknowledgement of receiving the Notice of Privacy Practices does not by itself prevent treatment under HIPAA. The provider should document that it made a good-faith effort to obtain the acknowledgement and record why it was not obtained. Other consent requirements may still apply, so staff should separate privacy notice acknowledgement from treatment authorization.

Who should own the new patient intake process?

A clinic manager or registration lead should own the intake workflow and completion standard.
